The problem is with Preview and how it corrupts PDFs. Once the damage is done it cannot be undone. Apple doesn't seem interested in fixing this longstanding problem.
This is only in Mac or it appens also in windows? I had a similar complaint that I could not diagnose.

Preview is only available on the Mac. The problem on Windows was probably due to not using Reader 11 or Acrobat. The buttonImportIcon method was (re)introduced to Reader with version 11, so it's not available in versions from 6 to 10.
A little grammatical error .
When I try to download a file, the box ask what do I want to open it with.
I selected the adobe type icon. It's a zip program or something, it
will unzip then gives a error message. I will have to do it to get the
error message.... :D

Download the file to the local disc and use File > Open in Adobe Reader or Adobe Acrobat.

I have a "To Date" field that presents an error message if the date entered into it is more than 3 months from the date contained in the "Date" field. The calculate custom script is below and it works fine.
I would like to add some additional script that presents an error message if the date entered in either the To Date field or the Date field is a past date. All dates entered must be after the dates in the both fields.
Can anyone assist me with the extra code to fit in with the code below

Adobe Reader users can only submit data formats such as FDF, XDP, XML, XFDF. PDF forms with extended usage rights enabled, can submit whole PDF format. I recommend setting the submit button to a URL pointing to a server-side script such as PHP or ASP.net, which sends the submission without client-side email software. Anyone with Adobe Reader can submit to a server-side script and send the form. Another way is to create a normal button, and set the URL to a mailto: email address, but this requires client-side email software or web mail to be setup correctly or it fails.

I recommend giving up on the idea of PDF forms in your case and use SECURE web forms (with professionally developed server software, DO NOT TRY TO DO THIS YOURSELF or you will be hacked).
Is there a way to make a button so someone entering information in a fillable form can go back to the link that sent them to a new location in another form or the same form?

Guess no one else has this issue...
I've already uploaded it to the server and can access it without problems, but cant sign it o submit it... I've created a button with a mailto action, but it wont work, can someone help me? When I try it on my local pc it works fine, the problem is online, can't sign nor submit... any idea?
It is probably not opening in Reader/Acrobat when you load it in a browser. Chrome and Firefox have built-in PDF viewers that have limited or no support for forms and other interactive elements. Reader can be configured to be the default in-browser PDF viewer, but it is up to the user to do this.
So is there any solution? How can I be assured that the user would be able to submit the form? Sometimes the users have no access to a personal pc and fill this form on public pc's, so I cant force them to download the file fill it and submit it back to us... any help?
You can't be assured of that. They may not have any PDF viewing software installed at all. And if they are using a public PC, no suitable email client. An alternative is not to use PDF and use an HTML form instead.
George, one more question and thank you again for being so responsive. I went ahead and purchased FormsCentral Plus but still see the same Forms Central interface when I open up FormsCentral. Am I missing a step in terms of activation?
I've used this PDF to HTML5 Converter with limited success. Had to user Dreamweaver to edit button functionality. Let me know if I was totally off track.
Do you have Windows Explorer using the Preview option?
Preview requires opening the file even it is display the preview Jpeg in the meta data. Turn off the preview mode and then delete the file. On a network you maybe butting heads with system backup or virus scanners.
