Adding Dynamic CC addresses

Hi everyone,

I have a wonderful form that works great:

There is one thing I am unable to figure out.  When the user clicks the "email form" button, I would like the email addresses entered in the two boxes at the top of the form, to be included as CC addresses when the email is sent.  Is this possible?

I've read this but haven't been able to make any of it work:

EDIT:  I should mention that this form uses the "send complete PDF as attachment" feature
