I don't see the behavior you're describing. Which specific field in that form is not working right and exactly what versions of Acrobat or Reader have you tested with? Can you post a screen shot showing the text piling up? It could be that the software you're using to create the fields is doing something incorrectly that causes the behavior you're seeing, but without more information, it's hard for me to track down the exact cause.
There are no fields in the "Reason for Rejection" area, but there are some typewriter annotations there. Also, using a version of Arial for text fields is usually not recommended. When you do, it unnecessarily increases the size of the file since the entire font gets embedded. When you specify Helvetica instead, Acrobat/Reader end up using a private version of Arial anyway as it is considered a suitable replacement for Helvetica. Conforming PDF viewers are guaranteed to have the base-14 fonts (Helvetica, TImes, Courier, etc.) available.
