If your receptionist has Acrobat (Standard is sufficient), she can export the data from the form that the client submits and import it into the second form. As long as the like fields share the same name, they will be populated when the data is imported into the form. She can then complete any other fields and submit to you. If she won't have access to Acrobat and instead just has Reader (11), there are other options, but they are a bit more complicated to set up.
