I advise you to forget PDF forms given your constraints and use HTML forms. What are you trying to gain by using PDF forms (notice that almost every form you meet on the web is an HTML form)?

It shold not occur if the user uses Adobe Reader or Acrobat. Non-Adobe PDF viewers (such as Preview) may not have as complete support for PDF forms and other features such as JavaScript.
