The other alternative is to reformat the .pdf as a webform and post it on FormsCentral. Unfortunately, that means one has to take the time to retype an existing .pdf form (and format the text fields) into a blank web form inside that system. It would be nice if one could just cut and paste existing fillable .pdfs. Shrug.
