Have a look at Adobe FormsCentral. It's a web-based form creating and hosting service can send you the results directly, without needing to do it over email.
If you really want to use PDF then just use the built-in Submit Form command, which uses mailto, but does work with web-based email services, like Yahoo, Gmail, etc. (only if you're using Acrobat/Reader XI, though).
