If you're collecting patient data in the US, you need to be aware of the HIPAA privacy and security rules. Email is generally not secure, especially if you need to collect data from the general public. FormsCentral is a secure method for submitting form data, but is not HIPAA compliant. It's easier to simply instruct the patients to fill-in and print. That's what I'm doing for my appointment on Tuesday. Savvier users might be convinced to bring in the filled-in forms on flash drives, but that's not without security concerns either.
